Guest rooms: Critical Care Ed has reserved rooms for participants and their guests at special rates in effect, based on availability from December 9 through the night of December 10 with December 11 checkout: $124 single, $134 double occupancy plus tax on all rooms each night. All guests shall make reservations directly to the hotel at 847-297-1234. Please request the room rate for Critical Care Ed. Reservations must include first night's deposit to be guaranteed. Be sure to request written confirmation of you registration.

Release Date: The hotel room release date is November 18, 2008. To take advantage of the reduced rates you must make your reservations before this date. Call hotel for availability after this date.

Room Cancellations: Contact the hotel regarding cancellations.

Course Cancellations: Cancellations may be faxed in writing. Cancellations received 7 days before the course will receive a full refund. Cancellations received after this time will be assessed a $20.00 processing fee. No refunds are given for "no shows" or cancellations received after the conference begins. A $20.00 fee is charged on any returned check. Your check must be received within ten days if you register by phone or fax or your registration may be cancelled.
