Registration and Payment Information

Registrations must include full tuition payment to be accepted. Conference registration and hotel rooms are limited, so early registration is advised. Make payment by check payable to Critical Care ED. A $20.00 fee is charged on any returned checks.

Phone, Fax and Online Registration
You may register by phone to reserve a spot by calling 217-787-5937 or you may fax registrations to 217-787-6020. Your check must be received within 10 days or your registration will be cancelled.

Cancellations
Cancellations may be faxed in writing. Cancellations received two weeks prior to program will receive a full refund. Cancellations received after this date are assessed a $20 processing fee. No refunds are given for "no-shows" or cancellations received after the conference begins.
